Prostaglandin analogues and ethacridine lactate for 1st and 2nd trimester induced abortion

Abstract

OBJECTIVE: To determine the efficacy of prostaglandin analogue, Misoprostol and Ethacridine Lactate for 1st and 2nd trimester pregnancy. DESIGN: A randomized controlled trial SETTING: Mahatma Gandhi Institute of Medical Sciences, Sevagram STUDY POPULATION: Women desiring medical termination of pregnancy METHODS: 120 women desirous of medical termination of pregnancy were distributed randomly into two groups. In group A (n= 42) Ethacridine lactate 10ml/week was instilled extraamniotically after adding 200 ug Misoprostol to it. Misoprostol 200ug was inserted intravaginally 2 hourly upto 4 doses. Group B (n= 78) received only 200 ug Misoprostol tablet intravaginally 2 hourly upto maximum 5 doses. Complete expulsion of products proved by ultrasonography was considered the end point. RESULTS: Second trimester pregnancies (of both groups) aborted completely within 7-48 hours. None of the cases required reinstillation of Ethacridine lactate. In 1st trimester 95% aborted completely in group A. 2 cases (5%) both below 8 weeks gestation required suction evacuation whereas in group B 89.7% aborted completely while 10.3% belonging to 1st trimester required suction evacuation. CONCLUSION: Misoprostol has the ability to interrupt pregnancy at any gestational age and addition of Ethacridine lactate enhances the efficacy of Misoprostol for 1st and 2nd trimester induced abortions without undue complications
